What to bring to your first appointment
Your first appointment allows us to get to know you. It is important that we understand your medical history and previous care you have received so we can provide you with the most appropriate care plan. Please tell us:
- Your Story
Tell us your story and don’t leave anything out. - Your Goals
What do you want to accomplish? - Your Medical History
Meet with one of our patient benefits representatives. - Your Medications
Please bring a complete, up-to-date list of medications you take. You can also bring the bottles or packages with you. The list of medications should include any herbs and supplements you take. - Insurance and ID Cards
Please bring your insurance and photo ID, such as a driver’s license. If your insurance company requires a referral for your first visit, please make sure your referring doctor has provided this. - Copay
Copays are due at the time of service. - Family History
It is important for us to know your family history to create the best treatment plan for you. - Emergency Contact Information
Bring a friend or family member
If visiting our facility for Oncology services, we allow one visitor over the age of 16 to accompany you. We highly recommend bringing a trusted friend or family member to assist you with taking notes during your visit.
- Meeting your physician, nurse, and other members of your care team
- Tests needed for an accurate diagnosis to guide your treatment plan
- Meeting with one of our patient benefits representatives to discuss insurance and billing questions, to try to lower your costs
Before leaving our office, our team will make sure to schedule you for any future visits if necessary.
As we transition from NMCCA to Nexus Health, it will be required for blood draw testing to be scheduled one day prior to your office visit with your provider. Patients can use any laboratory of choice. As of February 2023, patients will be able to have their labs drawn at the Nexus Health facility with TriCore Reference laboratory.
